Reporting Procedures for New Scholar, Senior Scholar, Infrastructure, New Initiative and Training Awards

Annual and Final Progress Reports MUST be submitted online at: http://www.cybergrants.com/emf/progress_report. You will be provided with login instructions approximately two months before the end of each funding year (in the case of annual reports for continuing awards), or approximately one month before the award end date (for awards in the final year of funding).

Annual Reports

Annual reports should be submitted before the 12th month of each funding year and consist of a scientific report, financial report (including a budget project for the coming year), and any updated assurances, if required. The annual scientific report should describe the progress made on the objectives in your proposal for the past funded year and objectives for the next year of the project (no specific format required). IMPORTANT: Be sure to use the appropriate financial report forms for your award (which may be downloaded from the progress reporting website, or below) and complete BOTH SHEETS of the Financial Report (which can be accessed by clicking on the tab buttons in the lower left corner of the Excel application window). An original hard copy of the financial report, signed by the investigator and an authorized financial officer, should be sent by mail to The Ellison Medical Foundation, 104 E. Final Reports

Final Progress reports are due within 60 days of the completion of an award. Final reports consist of a scientific progress report, financial report, brief lay language summary of your project for posting on our website, and any publications arising from the research project. The final scientific report should describe progress made over the entire funding period on the research objectives in your proposal, as well as any additional findings not contemplated in the proposal (no specific format is required). In particular, since our Foundation aims to stimulate innovation, we would appreciate comments on how this work has moved the field forward and/or allowed you to move into a new area of research. Please attach reprints or preprints of all publications arising from this project and referencing The Ellison Medical Foundation funding. In addition to your scientific report, we request that you provide a lay language summary, of no more than 1-2 paragraphs in length, to be used on the Foundation website and for other public information purposes. IMPORTANT: Be sure to use the appropriate financial report forms for your award (which may be downloaded from the progress reporting website, or at the bottom of this page) and complete BOTH SHEETS of the Financial Report listing final year expenses and expenditures over the entire award period, indicating any unobligated balance. These sheets are accessed by clicking on the tab buttons in the lower left corner of the Excel application window. Reporting Procedures for Conference and Workshops Awards

Final reports are due within 60 days after the conference date. In lieu of a scientific report, submit a narrative summary of the results of the meeting (which may be substituted by an abstract book, workshop report, or other similar publication), along with a financial report detailing how The Ellison Medical Foundation funds were expended (by category) and indicating any unobligated balance.
